Its standard practice to add a slight bend in the arms when in Edit Mode, which is what Faceb is eluding to.
That said, you can quickly fix your problem by changing your Pole Angle to 180.
In Edit Mode you can add, scale, move, rotate the bones to make it your 'reference' pose armature. In Pose Mode you can rotate, move and scale (but not add) bones, since here you can only change the pose of the armature. So if you switch between Edit Mode and Pose Mode you see different poses, unless you reset the armature:
In Pose Mode, if you press AltR with a bone selected, that bone will be reset (when you select every bone, the complete armature will be reset), according to how the bones are set up in Edit Mode. But I'm not sure if this is what you mean :)
